1955 Model 500 - Paper Inside

I recently acquired a soft plastic Western Electric model 500 dated April of 1955. When the cover was removed I found a curled piece of paper inside. The paper has yellowed on the edges. It appears to that it was torn from a perforated page. Perhaps it is from a whole page of the same number, like an inspector would use to identify something that was inspected. In my experiences working with telephone of this vintage I have not seen this before. Can anyone shed some light on this item? I believe this phone to be original with all dated parts similar.
